Pattern 1: OCBs in CSF only (positive): Oligoclonal bands present in CSF only. Intrathecal IgG synthesis as seen in MS; Pattern 2: No OCBs seen (negative, policlonal): No oligoclonal bands in CSF or Serum. No intrathecal Ig synthesis; Pattern 3: Identical OCBs in both (mirror): Bands in serum mirror those in CSF. This suggests systemic Ig synthesis; Pattern 4: Identical OCBs in both with extra in CSF (more than): Identical bands in both serum and CSF with extra bands in CSF. Image demonstrates both intrathecal and systemic Ig synthesis. This is identical as it is seen in MS.

Oligoclonal bands are found in electrophoretograms of serum of patients with multiple sclerosis.
A recent paper (1) describing oligoclonal banding in CSF indicates that it is necessary to electrophorese paired CSF and serum specimens simultaneously, to differentiate oligoclonal banding caused by intrathecal iinmunoglobulun production from increased permeability of the blood-CSF barrier. This dogma is not correct, as it has been shown by use of polyacrylaimde gel isoelectric focusing and Co...

Controversy exists regarding the definition of oligoclonal bands in cerebrospinal fluid (CSF) and whether CSF bands with corresponding bands in the serum should be disregarded in the interpretation. Much of this controversy results from not distinguishing between the sensitivities of different techniques used for these studies. Because the combination isoelectric focusing/silver staining is mor...

We assessed the frequency of cerebrospinal fluid (CSF) restricted oligoclonal IgG bands (IgG-OCB) in Portuguese multiple sclerosis (MS) patients and its relationship with outcome. Paired CSF/serum samples of 406 patients with neurological disorders were submitted to isoelectric focusing with immunodetection of IgG. Ninety-two patients had definite MS; non-MS cases were assembled in groups infla...

The presence of oligoclonal bands (OCBs) of immunoglobulin G (IgG) in CSF provides evidence for the occurrence of a humoral immune response, but it is not always appreciated that the oligoclonal IgG may have originated in the serum. To determine the diagnostic significance of serum OCBs 146 patients with serum OCBs were identified among 1874 patients with suspected neurological disorders (7.6%)...
